General comments
Criteria voor verhoogd risico ESBL: infectie hospital-acquired/health-care associated, recent (<2 maanden) gebruik breed-spectrum antibiotica, recent (< 3 maanden) reis naar hoog-endemisch gebied (zie https://resistancemap.cddep.org/) en recente (< 2 maanden) ziekenhuisopname in buitenland.
Denk aan spiegel controle (TDM) indien verwacht >2 giften gentamicine
Indien gentamicine vaker dan één keer wordt toegediend:
Bij gebruik van gentamicine moeten spiegelbepalingen worden verricht.
Gentamicine: bij obesitas (=BMI>30 kg/m2): om toxiciteit (m.n. nefro- en neurotoxiciteit) bij obese patiënten te voorkomen, doseer op basis van het “adjusted body weight”
