General comments

Gentamicine  
Toevoegen van 2 weken gentamicine 3mg/kg/dag mag kan overwogen worden

Behandeling met gentamicine dient gestopt te worden bij tekenen van toxiciteit

Indien gentamicine vaker dan één keer wordt toegediend:

  • Overleg met consulent medische microbiologie of internist-infectioloog
  • Bij gebruik van gentamicine moeten spiegelbepalingen worden verricht. Controleer een dalspiegel voorafgaand aan de 2e gift
  • Bij obesitas (BMI≥30 kg/m2): om toxiciteit (m.n. nefro- en neurotoxiciteit) bij obese patiënten te voorkomen, doseer op basis van het adjusted body weight
